The MAT Grant Program is only available for
training taken in 2008.
The SUNY Medication Administration Training Grant
Program is funded by the New York State Office of Children and Family
Services and is administered by the Research Foundation of SUNY
Training Strategies Group.
If you plan to take the MAT
Course, you can receive up to $100 to help pay for the course
through the MAT Grant Program.
Can I apply for MAT Grant funds?
You can apply if you work in New York State, have not received MAT
Grant funds in the past three years and are:
- A registered family child care provider currently caring for
children
- A licensed group family child care provider currently caring
for children
- A director, authorized representative or staff of a child care
program currently caring for children in a registered school-age
program
- A director, authorized representative or staff of a child care
program currently caring for children in a licensed small day
care center
- A director, authorized representative or staff of a child care
program currently caring for children in a licensed child care
or Head Start center
- A legally exempt child care provider serving children whose
child care is being funded through public child care subsidies
If you plan to take training in 2008,
go to the link on the right under the “MAT Grant Downloads” section
and click on the 2008 application. Your application
must be received no later than December 31, 2008.
How do I fill out the
2008 application?
If you are planning to take the MAT course in 2008 and you
(or your program) have NOT paid for your training:
- Complete Part 1 of the application, Sections A and B.
- The owner, director or authorized representative at the program
where you work must sign under Section B. If you are a legally
exempt provider, fill out Sections A and Part B for legally exempt
providers only. If your application is not filled out completely,
it will be returned to you.
If you are planning to take the MAT course in
2008 and you (or your program) HAVE paid for your training:
- Complete both Part 1 AND Part 2 of the application.
- Complete the receipt section or attach your original receipt.
You can have the MAT Trainer or Training Organization fill out
the receipt printed on Part 2 OR you can attach your original
receipt which must include the date you paid, amount you paid,
who paid for the training, receipt number and an original signature
from the person who received your payment.
- Complete the W9 printed on the back of the application.
- If you paid for the training, fill out the W9 following the
directions. NOTE: If you fill out the “Business Name” section
of the W9, the check will be payable to that business name so
only include that name if you are able to cash checks written
payable to your business name.
- If your program paid for you, have the program fill out and
sign the W9.
What happens after
I apply?
If you are eligible for funding and have not paid for your
training, you will receive a MAT Grant Voucher for $100
within 4-6 weeks. The voucher will expire 60 days
from the date printed. If the training date will occur after the
expiration date, contact the MAT Grant Program. You can give the
voucher to the MAT
Trainer as payment toward the training. Please allow 8-10 weeks
for processing.
If you are eligible for funding and paid
for your training, please allow 8-10 weeks to receive your
rebate check.
If you are not eligible for funding, you will
receive a letter from the MAT Grant Program.
